Applications for Fall 2012 will be accepted beginning on January 17, 2012 and the application window will close on April 2, 2012. Review of applications will begin shortly after the application period opens due to the rolling admissions process, we would strongly advise interested applicants to complete their application as soon as possible. Please note, your file will go forth for Committee review once all material stated on your application are received.


Please note that the Fall 2012 cohort will be the only cohort accepted during the 2012-2013 academic year. We anticipate accepting 24 students for the Fall 2012-2014 cohort.

Fall 2012-2014 application for the Pre-Health Professions Certificate

One set of official transcripts from each college or university you attended.
Note: Due to the time constraints for timely receipt of official transcripts, we now require all transcripts to be uploaded in a PDF format to http://www.jotformpro.com/hppostbac/cohort5. However, please be aware that Admissions and Records will still require official paper copies.

Completion of FAFSA Application for Financial Aid

Completion of, or in progress, course in Chemistry that is equivalent to CSUF’s Chem 115. This course must be completed by June 15, 2012 in order to prevent your application from being dismissed due to the lack of pre-requisites.

One letter of recommendation (LOR) from a health professional or science faculty member; a maximum of three letters will be accepted. For more information, please download the LOR Guideline.
